Preparation for IVF Cycles – Testing and Ovarian Stimulation

To ensure that there are no problems, IVF will be performed before you can have IVF. A pre-cycle test includes a hormonal evaluation to determine thyroid function and ovarian reserve. It also screens both partners for sexually transmitted infections.

The average time for ovarian stimulation is between 8-14 days for most women. However, the majority of women will only take fertility drugs. It takes about 10-11 days on average. For egg retrieval, ovarian stimulation is used to mature multiple eggs. Ovulation can be normal but fertility drugs can be used to produce multiple eggs. Pregnancy rates are higher when there are more eggs. For IVF, an average of 10-20 eggs are typically retrieved. However, not all eggs are suitable for IVF as only two-thirds of them have reached the right maturity.

Your doctor will carefully plan a protocol that will allow you to get the most eggs and protect against ovarian hyperstimulation syndrome (OHSS). IVF drugs are often injected. You’ll also be regularly monitored with hormonal testing and vaginal ultrasounds to ensure the best results. An ultrasound will determine if you have enough follicles and if your estrogen levels are at the correct level. You’ll be given a trigger shot with hCG or another medication. This is the replacement of the natural luteinizing hormone boost a woman receives that stimulates the final stage of egg maturation. Eggs can then be fertilized.

Egg Retrieval

You will need to have surgery to remove eggs from your follicles between thirty-four and 36 hours after you receive the trigger shot. An ultrasound is used to guide a needle through the top vaginal wall into the ovary. The procedure will not cause any discomfort or pain as you will be administered IV sedation and closely monitored by an Anesthesiologist.

The needle is used to insert the follicles. The fluid contains the follicular fluid and is removed by gentle suction. This usually takes no more than 30 minutes. Some minor cramping may occur the day before the procedure, but this is usually resolved by the next day. You may feel fullness or pressure from the ovarian stimulation. This sensation may last several weeks.

The IVF physician then suctions the fluid from the follicles, which contain the egg. An embryologist then examines the test tube and finds the egg inside. The details of each egg are meticulously recorded. The number of eggs produced or removed depends on the patient’s age, ovarian reserve, and response to ovarian stimulation. Sometimes, it is possible to access the ovaries with a needle.


Experts inspect eggs once they reach the laboratory to assess their maturity and quality. After maturation, eggs are placed in special media and then fertilized with sperm within hours. You can fertilize an egg with an intra-cytoplasmic injection or conventional insemination. Your IVF team (embryologists and doctors) will decide which process to use. This depends on many factors that are relevant to the couple who is undergoing IVF. Both methods have roughly the same success rate. ICSI can be used 70% of the time when fertility is less likely because of poor semen quality or IVF failures.

The conventional method involves sperm being placed in a culture medium and an egg in a small dish. After the eggs and sperm are incubated in the laboratory, the sperm can enter the egg by itself. One sperm is used for ICSI. It is injected into an egg’s cytoplasm using a needle and a sophisticated operating microscope. No matter what process was used fertilization is monitored the next day.

Embryo Transfer

The IVF team and the couple decide when embryo transfers will occur. This can be anywhere from 1 to 6 days, but most often within 3-5 days of egg retrieval. If genetic testing is required, the IVF team and the couple must first take a biopsy from the embryo. This is almost always done on culture day 5 or 6. The IVF laboratory will send 3-8 cells to an outside lab for testing. The embryos remain frozen. The genetic test results are received and the embryo chosen is thawed, then transferred to the uterus. This usually happens within one to two months.

The couple’s age and the number of embryos they produce will play a role in the number of embryos that are produced. Multiple embryos were sometimes transferred to maximize success in the past. However, this could lead to twins or rare triplets. Both of these complications can be fatal for both mother and baby.

Limiting embryo transfer to one embryo is the best way to keep your baby safe. The embryologist selects the most healthy embryo based on the grading system that was used to assess each embryo. This maximizes the chances of success.

The embryo is transferred into the uterus using a thin, flexible and soft catheter. To ensure that the catheter’s tip places the embryo in the most suitable place for it to the implant, an abdominal ultrasound is performed. The experience is similar to a pap test. There is rarely any pain or discomfort. If a cycle fails or a couple wishes to have more children, good embryos are often frozen.

The embryo develops in the uterus, and hatches and implants within the uterine membrane.

Pregnancy Test

A blood pregnancy test will be done approximately 12 days after embryo transfer. You’ll have blood tests, and then an ultrasound to confirm a pregnancy. Your obstetrician will refer you back if the pregnancy is normal after 9-10 weeks.
